3. CareLink Referral Submissions

CareLink by Home Care Placement allows authorized referral partners to submit referrals for individuals who may need care, support, placement, or related services.

Referral partners are responsible for ensuring that they are authorized to submit information about an individual and that their submission complies with applicable privacy, confidentiality, healthcare, and other legal requirements.

CareLink referral forms are provided through Jotform. CareLink forms that may collect protected health information use Jotform's HIPAA-enabled features.

Submission of a CareLink referral does not guarantee acceptance of the referral, availability of services, eligibility for services, or placement with any particular provider or community.

4. Senior Living Placement Information

Information regarding senior living communities, care settings, or placement options is provided to assist individuals and families in evaluating potential options.

Home Care Placement does not own or operate the independent third-party senior living communities that may be discussed or recommended through our placement services.

Information concerning a community's services, amenities, pricing, availability, licensing, policies, staffing, or other characteristics may change and should be independently confirmed with the applicable community before making a decision.
